Where to begin?

Home health care is usually initiated by a hospital or rehabilitation center social worker to make the transition from rehab to home as smooth as possible. They will meet with the patient and caregiver to discuss any questions or concerns.

As a caregiver, you can help by asking important questions:

  • What are your loved one’s wishes and preferences?
  • Do you know where all important documents and passwords are kept including bank accounts, retirement accounts, safe deposit boxes, investments, wills and trusts?
  • Who else can help with caring for your loved one?
  • What kind of care does your loved one need now and how might that change in the future?
  • Does your loved one have a long term care insurance policy? Does he/she need it?
  • Does your loved one have any advanced directives in place?
  • Does your loved one have any power of attorney?
  • Does your loved one have a will?
  • Has your loved one communicated any final wishes for after his/her death?

To Qualify for Home Health Care

Life Fountain provides skilled care as ordered by your physician. Skilled care is provided on an intermittent basis (visits) for patients confined to home due to illness or injury or who are recovering from surgery.

To qualify for home health care (HHC), you or your loved one must meet the following criteria:

  • Be considered “homebound,” based on the Centers for Medicare & Medicaid (CMS) criteria:
    • Require skilled care on a part-time or intermittent basis to improve, maintain, prevent, or further slow your health condition,
    • Be under the care of a doctor, who completes and documents an in-person visit with you either:
      • three months before the start of HHC, or
      • within one month after the HHC benefit has begun.
      • Receive HHC from a Medicare-approved home health agency.

Methods of Payment:

  • Medicare – 100% coverage

 

Medicare:
Medicare Part A (Hospital Insurance) and/or Medicare Part B (Medical Insurance) covers the cost of eligible home health services. Types of home health services covered include:

  • Part-time or intermittent skilled nursing care
  • Physical therapy
  • Occupational therapy
  • Speech-language pathology services
  • Medical social services
  • Part-time or intermittent home health aide services (personal hands-on care)
  • Usually, a home health care agency coordinates the services your doctor orders for you.

 

Medicare doesn’t pay for:

  • 24-hour-a-day care at home
  • Meals delivered to your home
  • Homemaker services
  • Custodial or personal care (like bathing, dressing, or using the bathroom), when this is the only care you need

Once your loved one arrives home, a Life Fountain RN will visit to assess needs and begin the process of building the best team to provide the greatest benefits of health, healing, and wellbeing.

Skilled Nursing

Upon your arrival home our nurse will assess and determine a comprehensive care plan based on your needs. The nurse maintains communication with your physician for enhanced treatment of wound care, high-tech infusion, Foley care, diabetic management, skilled observation and assessment, medication monitoring and many other nursing procedures.

Physical Therapy

Registered Physical Therapists and Physical Therapy Assistants address treatment techniques to promote the ability to move, balance, reduce pain, restore function and reduce further injury. Therapists assist and/or train patients with gait training, use of wheelchairs, walkers, artificial limbs, etc., and pain control.

Home Health Aide

Certified Nursing Assistants maintain communication with your physician, and assist patients with activities of daily living and personal care.

Occupational Therapy

Occupational Therapy addresses the skills needed to complete the activities of daily living. Activities may include eating bathing, grooming, dressing, and toileting to promote independence.

Social Worker

Social Workers assist patients in solving problems that may arise through illness, including social, financial and transportation problems.

Speech Therapy

Registered Speech Therapists provide an effective speech program assisting patients in learning to better use and comprehend language. They provide intervention with swallowing difficulties and enhance written, spoken, and nonverbal communication.
