Home health care is usually initiated by a hospital or rehabilitation center social worker to make the transition from rehab to home as smooth as possible. They will meet with the patient and caregiver to discuss any questions or concerns.
As a caregiver, you can help by asking important questions:
Life Fountain provides skilled care as ordered by your physician. Skilled care is provided on an intermittent basis (visits) for patients confined to home due to illness or injury or who are recovering from surgery.
To qualify for home health care (HHC), you or your loved one must meet the following criteria:

Upon your arrival home our nurse will assess and determine a comprehensive care plan based on your needs. The nurse maintains communication with your physician for enhanced treatment of wound care, high-tech infusion, Foley care, diabetic management, skilled observation and assessment, medication monitoring and many other nursing procedures.
Registered Physical Therapists and Physical Therapy Assistants address treatment techniques to promote the ability to move, balance, reduce pain, restore function and reduce further injury. Therapists assist and/or train patients with gait training, use of wheelchairs, walkers, artificial limbs, etc., and pain control.
Certified Nursing Assistants maintain communication with your physician, and assist patients with activities of daily living and personal care.
Occupational Therapy addresses the skills needed to complete the activities of daily living. Activities may include eating bathing, grooming, dressing, and toileting to promote independence.
Social Workers assist patients in solving problems that may arise through illness, including social, financial and transportation problems.
Registered Speech Therapists provide an effective speech program assisting patients in learning to better use and comprehend language. They provide intervention with swallowing difficulties and enhance written, spoken, and nonverbal communication.
